INTERPRETER SERVICES

In order to ensure effective communication between patients and staff; People’s Center provides qualified interpreters free of charge for those with limited English proficiency and for those challenged with a hearing impairment. Patients seeking interpreter services may notify our call center staff when scheduling an appointment, and/or may notify front desk staff for walk-in appointments.

CHRONIC DISEASE MANAGEMENT PROGRAM

Managing chronic diseases such as diabetes, hypertension, and depression require a team effort. To help each patient improve their health and quality of life, our staff assign's each patient to a care team that helps them learn how to best manage chronic illness and create an individualized, realistic, and culturally responsive plan for future action. Our care management team may include a primary care provider, community health worker, nutritionist, behavioral health professional, dental provider, and pharmacist. CASE MANAGEMENT

Our clinical social work and care management staff are able to connect patients with those community resources that are most helpful and relevant for you and your family. These include, but are not limited to, the following: housing, emergency assistance, legal guidance, disability assistance, home care services, State Medical Review Team (SMRT), insurance navigation, citizenship assistance, financial resources, Adult Rehabilitative Mental Health Services (ARHMS), and Metro Transit handicap services.
